Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

16-23-4    Basic - Accumulation of lime scale on the inside of the dishmachine.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. **Repeat Violation**
24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner.
35A-03-4    Basic - Dead roaches on premises. Observed 1 dead roach next to ice equipment, 1 in the pots and pans Shelves area floor, 1 underneath Dishwashing equipment floor 1 in front of walk-in cooler floor, 2 on top of dishwasher Machine.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed accumulation of debris underneath non working cooler, kitchen area.
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. **Repeat Violation**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed Approximately 15 live roaches in the kitchen underneath Nonworking reach in cooler next to Prep sink.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. **Repeat Violation**
11-07-5    Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items. **Repeat Violation**
