Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-37-5    Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Ceiling of the hood on the front line, extremely soiled and large amount of dust buildup.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. **Repeat Violation**
14-74-7    Basic - Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach in cooler drawers not holding temp of 41° of below, do not hold any tcs food items in unit until repairs have been completed.
36-03-4    Basic - Cove molding at floor/wall juncture broken/missing. Thru out the location, missing cove molding/ base boards.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Pizza station make table cutting board
13-04-4    Basic - Employee with no beard guard/restraint while engaging in food preparation. Added once they where notified that the health inspector was on site **Corrected On-Site**
14-11-5    Basic - Equipment in poor repair. Door gasket on upright reach in freezer in need of repair.
14-10-4    Basic - Equipment or utensils not designed or constructed in a durable manner. Floor stand mixer bowl no longer has stainless steel coating, underlaying metal exposed. **Repeat Violation**
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Floor in dough room behind the water heater in need of cleaning, old food debris . **Repeat Violation**
36-17-5    Basic - Floor tiles missing and/or in disrepair.
14-38-4    Basic - Food storage container/container lid cracked or broken.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Reach in freezer fryer items unit
36-62-4    Basic - Light not functioning. Multiple lights and fixtures not in working order they the location. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of equipment on the cooks line in need of cleaning. **Repeat Violation**
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior of the reach in cooler units in need of cleaning on cooks lines. **Repeat Violation**
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface.
21-10-4    Basic - Soiled dry wiping cloth in use. Dry soiled wiping cloths being used as both wiping cloth and as a pot holder.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Beer keg cooling holding standing water.
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. **Repeat Violation**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Behind the Dish Washing Machine **Repeat Violation**
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish Washing Machine machine at less then 10 ppm chlorine, primed and retested to 50ppm chlorine. **Corrected On-Site** **Repeat Violation**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. 1/2 quart heavy whipping cream, no date marked, spoiled.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Ambient air temp for unit reading 52°, reach in cooler drawers at back grill ; raw ground beef in unit since 12noon (50-51F - Cold Holding); sliced cheese in unit since 12 noon (50-51F - Cold Holding); raw shaved beef in unit since 12noon (50-51F - Cold Holding). Items placed into the reach in freezer to rapid chill to 43-44 within 25 mins. **Corrective Action Taken** **Repeat Violation**
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Heavy whipping cream at bar, chunky, not dated, no actual time of when it was opened, see stop sale. Heavy whipping cream on reach in cooler in wait station not date marked from opening on 2 days prior. **Repeat Violation**
11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Needed paperwork for newly hired/ on site employees not yet completed the food safety training in need of filled out paperwork. Needed form again emailed to operator to be complied by the next unannounced routine inspection.
02C-08-5    Intermediate - Ready-to-eat, time/temperature control for safety food opened/prepared, frozen and then thawed and held at refrigeration temperature not properly date marked. Fully cooked stuffed pasta shells and ravioli not date marked from when removed from walk in cooler. **Corrected On-Site**
53B-10-4    Intermediate - Records/documents for required employee training do not contain all of the required information. Multiple employee training cards without DOB.
