Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. 1 dead roach in area with fire department hook ups/emergency shut off, between main kitchen and employee bathroom. **Repeat Violation**
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of both ovens on first floor.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Sides of ovens and stoves on cook line in kitchen on first floor. **Repeat Violation**
35B-03-4    Basic - Outer openings not protected with self-closing doors. Back door to main kitchen does not have a self closing device.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Bulk containers of sugar and salt on main cook line not labeled.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. 2 dented cans of red peppers in dry food storage area in main kitchen. 1 dented can of coconut milk in dry food storage area in main kitchen.

03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cheese (45F - Cold Holding); chopped chicken (45F - Cold Holding) by door inside walk in cooler, per employee they have been going in and out of walk in cooler putting away a food delivery less then 1 hour prior.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Squeeze bottle with blue liquid stored with various condiment bottles at third floor kitchen., employee moved bottle. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Blade of can opener on prep table in main kitchen on first floor. **Repeat Violation**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Hand wash station in corner of prep area in main kitchen blocked with box of paper towels.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. At hand wash stations in main kitchen, employee corrected during inspection. **Corrected On-Site** **Repeat Violation**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Multiple squeeze bottles with blue liquid throughout main kitchen area.
