A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

Approximately 17 live flying insects in bar area. Approximately 5 live flying insects in beverage station by kitchen entrance. 2 live flying insects on cook line. Employee sprayed in bar, beverage and cook line. After a few minutes I rechecked and found approximately 5 flying insects in the bar area, 1 flying insect on the cook line, and approximately 8 flying insects in the beverage station by the kitchen entrance. **Corrective Action Taken** **Repeat Violation** **Admin Complaint** - From follow-up inspection 2023-02-08: 3 flying insects by soda rack in storage area near the cook line. **Admin Complaint**

Raw steak and beef stored over opened case of stuff jalapeno with cream cheese in walk in freezer. 03B-01-6    High Priority - - From initial inspection : High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. ground beef (110F); pulled chicken (109F); pulled pork (108-110F) in steam table. Cook stated food was reheated and placed in steam table for hot holding approximately 2 hours ago. Cook began reheating food. **Corrective Action Taken** **Warning** - From follow-up inspection 2023-02-08: At 6:00ppm found pulled beef 119-126F in steam table on cook line. Cook stated beef was reheated and placed in steam table to hold hot today at 3pm. At 6:00pm found cooked rice 122F inside secondary oven on cook line. Oven is not on. Cook stated rice was cooked and placed in oven as a back up at 3pm. Employee began reheating beef and rice. **Admin Complaint** **Corrective Action Taken**
03D-15-4    Intermediate - - From initial inspection : Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. At 3:30pm found cooked beans at 70-71F cooling, queso 70F cooling, cooked chicken 78F cooling at room temperature on cook line. At 4:10pm took second temperature of cooling food and the food is at the same temperature. Employee stated food was cooked a few minutes ago and placed at room temperature to cooled down. Employee moved food to walk in cooler. **Corrective Action Taken** **Warning** - From follow-up inspection 2023-02-08: Cooling not observed during inspection **Time Extended**
