A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices.

Basic - Observed Accumulation of black/green mold-like substance in the interior of soda nozzles at soda machine at dining room area and drive thru area.
Basic - Observed Employee drinking from an unenclosed beverage container in a food preparation cook line /clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container.
Basic - Observed Employee eating in a food preparation or other restricted area at cook line.

Basic - Observed Employee personal items cellular phone stored in or above a food preparation area, food, clean equipment and utensils, or single-service items at drive thru station.

Basic - Observed Reach-in cooler interior/shelves at cook line have accumulation of soil residues.
Operator had employee start cleaning cooler **Corrective Action Taken**
High Priority - Employee at bathroom used the bathroom and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands upon returning to employee area.
Observed employee return to work station from restroom and engaged in beverage preparation without washing hands.
Inspector coached employee and had employee wash hands and wear gloves.
Employee touched face/hair and then engaged in beverage preparation without washing hands.
High Priority - Presence of insects, rodents, or other pests.
Observed approximately 100 live maggots or more in standing water on floor moving around at cook line next to nacho chip holder.
Intermediate - Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items.
Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment.
