Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Twelve dead roaches on floor under four stainless steel freezers along wall of kitchen. Thirteen dead roaches under dish machine in kitchen. Two dead roaches on floor at high chairs located at sushi bar. Nine dead on floor in beverage area outside of kitchen. Roaches removed and sanitized areas. **Corrective Action Taken**
14-11-5    Basic - Equipment in poor repair. Gaskets to white freezer chest. Freezer chest lid torn.
36-73-4    Basic - Floor soiled/has accumulation of debris. Throughout kitchen.
08B-38-4    Basic - Food stored on floor. Vinegar bucket on floor in kitchen entry way. Avocados on floor at sushi bar. Onions on floor in storage room.
36-24-5    Basic - Hole in or other damage to wall. Holes in wall at kitchen entry.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Metal shelving unclean storing clean dishes and deli slicer. Hand sink in kitchen. Gaskets to two door freezer along wall in kitchen. Gaskets to large white freezer chest.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Fish roe thawing at room temperature
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Wall tiles behind cook line unclean. Walls at kitchen entry way.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Five dead flies in container storing styrofoam cups and lids. Discarded. Sanitized container. **Corrective Action Taken**
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. **Admin Complaint**
08A-02-6    High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Raw shrimp over chili peppers, raw scallops over cheesecake slices, raw pork over vegetable spring rolls in two door freezer in kitchen. Operator stored properly. **Corrective Action Taken**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Five live roaches in gasket of large white freezer chest in kitchen. Operator killed and removed roaches. **Corrective Action Taken**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Two stainless steel pans in hand sink at sushi bar.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. In kitchen. **Corrected On-Site**
