Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Clean storage pans stored on rack/shelf dry storage area.
14-11-5    Basic - Equipment in poor repair. Walk in freezer door is cracked and has ice build up around it.
36-17-5    Basic - Floor tiles missing and/or in disrepair. Tiles by walk in freezer door. Tiles by prep sink in back prep area.
36-73-4    Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. Floor under racks in kitchen. Floor in walk in freezer. Ceiling vents in back dry storage area.

Cook handled soiled pans with gloved hands near prep sink in kitchen and immediately returned to cook line and handled pan of cooked shrimp without washing hands. In flip top cooler on cook line across from grill, pans of raw beef and raw chicken stored over ready to eat tortillas, peppers and mushrooms. 03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On cook line near grill, white bucket of cut tomatoes 60F. 03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Near flat top grill on cook line, pan of cooked chorizo on shelf 108F. Chorizo cooked approximately 20 minutes prior. 02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Menu item ceviche is not identified as a raw product (to go menus). Manager had hostess mark and identify ceviche during inspection and will not give out menu to anyone without this identification. **Corrected On-Site** **Repeat Violation**
