Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed reach in cooler in the kitchen with heavy soil. **Warning**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Observed water leaking from pipe under the three compartment sink. **Warning**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. **Warning**
36-10-4    Basic - Worn, torn and/or soiled floors/carpeting. Observed floor under 3 compartment sink and back storage area soiled. **Warning**
03D-02-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed rice and bean mix (58F - Cooling), macaroni and cheese (62F - Cooling); cooked chicken (46F - Cooling); goat (46F - 52F - Cooling) stored in the reach in cooler over night, as per operator. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 30 flies on the ceiling by the back storage area on top of the mop sink.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach crawling on top of the hot water heater located in the back storage area.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 15 rodent droppings by the reach in freezer in the back storage room.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed rice and bean mix (58F - Cooling), macaroni and cheese (62F - Cooling); cooked chicken (46F - Cooling); goat (46F - 52F - Cooling) stored in the reach in cooler over night, as per operator. **Warning**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed Clorox wipes stored on top of prep area in the kitchen. Operator removed. **Corrected On-Site** **Warning**
27-06-4    Intermediate - No hot running water at three-compartment sink. Observed three compartment sink with hot water at 72F, ran water for about 2 minutes. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towel in both handwashing sinks in the kitchen. Operator placed new roll. **Corrected On-Site** **Warning**
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. The direct link to the Plan Review page is https://www2.myfloridalicense.com/hotels-restaurants/licensing/plan-review/. Observed the approved plans does not match the current layout of the establishment. Observed 3 compartment sink was changed to a 2 compartment sink, observed only 1 bathroom for customer use, and observed mop sink was removed from the outside. **Warning**
16-28-4    Intermediate - Two-compartment sink used for warewashing. **Warning**
27-16-4    Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed handwashing sink with hot water of 72F, ran water for about 2 minutes. **Warning**
