Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-11-5    Basic - Equipment in poor repair. Observed the reach in cooler gaskets torn at the bar next to ice bin.
08B-38-4    Basic - Food stored on floor. Observed a plastic container of cooking oil plastic water bottles and juice bottles on the floor by the stairs.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed 5 tongs on the oven doors. The chef removed the tongs. **Corrected On-Site**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the reach in cooler gaskets soiled at the bar.
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface. Observed in the reach in cooler with the meat slicer is on top.
25-05-4    Basic - Single-service articles improperly stored. Observed several cases of Togo containers on the floor upstairs.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. **Repeat Violation**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed a container with sugar and a container with salt not labeled on top of the reach in cooler on the back wall. 12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food.
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. The owner claims to have paid during the inspection confirmation number 236035829. **Corrective Action Taken**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed a spray bottle with cat repellent on top of orange juice bottles next to the stairs. The owner removed the chemical. **Corrected On-Site**
01C-05-4    Intermediate - Clam/mussel tags not maintained in chronological order according to the last date they were served in the establishment.
01C-03-4    Intermediate - Clam/mussel tags not marked with last date served. **Repeat Violation**
01C-10-4    Intermediate - Clams/mussels/oysters tag removed from original container prior to container being emptied. Observed in each in cooler drawer at the cook line. The cook found the tag and put it with the clams. **Corrected On-Site** **Repeat Violation**

11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses.
