Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation in the interior of the ice machine/bin. Lime build up in upper interior of ice machine located adjacent to storage area near rear exit.
35A-06-4    Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed flying insect sticky tape with accumulation of dead insects. Person in charge switched. Also, observed one dead fly on container with noodles. Person in charge removed and changed out container. **Corrective Action Taken**
16-23-4    Basic - Accumulation of lime scale on the inside of the dishmachine. Lime buildup observed in dish machine.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee cell phone sitting atop shelf on prep table adjacent to entrance.
14-11-5    Basic - Equipment in poor repair. Reach in coolers along cookline and near ice machine near rear exit have torn/frayed gasket seals. White refrigerator in rear adjacent to triple sink has significant ice buildup. Ice machine and ventilation duct located in rear of kitchen near exit door are both heavily duct taped on lid and ventilation opening.
36-73-4    Basic - Floor soiled/has accumulation of debris. Floors beneath cooking line and along prep table, triple sink and storage area near rear exit is heavily soiled with grease, food debris.
36-24-5    Basic - Hole in or other damage to wall. Observed hole in wall by rice cooker in kitchen. Hole in wall in office. Hole in wall by outlet by ice machine. Hole in wall to left of sliding cooler in the bar.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Reach in coolers along cookline and near ice machine next to exit have heavily soiled gaskets. Reach in cooler near cookline has moldy exterior. Hood filters and hood with heavy grease buildup.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed chicken thawing at room temperature in kitchen. Person in charge moved to cooler. **Corrective Action Taken**
36-04-5    Basic - Utility lines are unnecessarily exposed. Ceiling tiles left and right of cooking station are in disrepair and utility lines/plumbing are exposed.
36-10-4    Basic - Worn, torn and/or soiled floors/carpeting. Floors beneath cooking line and along prep table, triple sink and storage area near rear exit is heavily soiled.
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Observed 4 dented cans of bamboo shoots on storage rack to the rear of the dining facility heavily dented. Also, one container of prepped garlic with mold like growth.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 2 flying insects near triple sink in kitchen. Observed 4 flying insects under triple sink off of kitchen. **Warning**
08A-04-5    High Priority - Raw animal food stored over or with unwashed produce. Raw shell eggs over cucumber in white refrigerator adjacent to prep line. Also, raw chicken not commercially packaged stored over ready to eat imitation crab in white freezer adjacent to prep table.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 90 rodent droppings on floor and under storage nook to the right of cookline. Observed approximately 3 rodent droppings on a dinner plate on the shelf of storage nook. Observed approximately 25 rodent droppings on the floor in an adjoining nook on the other side of wall. Observed approximately 5 rodent droppings to the right of the white freezer in storage area. Observed approximately 6 rodent droppings under desk in office off dining area. Observed approximately 10 rodent droppings behind hostess stand at entry of dining room. Person in charge began cleaning. **Corrective Action Taken** **Warning**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. At front hand wash sink observed towel in sink, also observed ice in sink.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink behind service counter or hand wash sink inside kitchen adjacent to triple sink. Person in charge provided roll during inspection. **Corrective Action Taken** **Repeat Violation**
31B-03-4    Intermediate - No soap provided at handwash sink. No soap at hand wash sink inside kitchen adjacent to triple sink.
03F-10-5    Intermediate - No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Person in charge holds garlic in oil, soaked rice noodles, cut cabbage, cut tomato, rice on time, no time paperwork available during inspection. Emailed to operator. **Corrective Action Taken**
41-04-4    Intermediate - Toxic substance/chemical on premise that is not required for the operation of establishment. Observed a bottle of medicine on the left side shelf of ice machine in rear near exit.
