Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of ice machine bin soiled with accumulation of black/green mold-like.
35A-06-4    Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed accumulation of dead flies on controlled device.
23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles. Observed all reach in cooler doors in kitchen soiled with food debris.
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tile in disrepair at storage room.
06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed thawed two packs of raw fish in walk in cooler not removed from reduced oxygen package. Coached employee on proper thawing procedures. **Repeat Violation**
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed cut marks on cutting board in kitchen.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed personal beverage on top on the oven in kitchen.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed phone, car keys, and phone chargers stored in storage room shelves and dishwashing area.
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed exterior door has a gap at sides of door near dish washing station.
36-73-4    Basic - Floor soiled/has accumulation of debris. Floor soiled with food debris inside walk in freezer.
08B-47-4    Basic - Food not stored at least 6 inches off of the floor. Observed water bottles near restroom not stored 6 inches above ground.
08B-38-4    Basic - Food stored on floor. Observed container of ice and container of raw shrimp on floor inside walk in cooler. Observed water bottles near restroom not stored 6 inches above ground. **Repeat Violation**
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on floor of walk in freezer. **Repeat Violation**
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed tongs stored on equipment handle on cook line.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hood filters soiled with grease above wok station. **Repeat Violation**
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed interior shelves of reach in cooler shelves soiled with food debris near storage area.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed walls soiled with grease and food debris near the hand sink at ware washing area, by the ice machine, and dumpling station.
29-03-4    Basic - Water draining onto floor surface. Observed water draining onto floor from the steamer in kitchen and water draining from underneath hand wash sink onto floor beside 3 sink compartment.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw chicken washed vegetables in walk in cooler. Manager moved chicken and placed in in the correct order. **Corrected On-Site**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed thawed two packs of raw fish in walk in cooler not removed from reduced oxygen package.
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed egg custard (112 F - Hot Holding). At hot holding cabinet at preparation area. As per Manager, egg custards were placed there 20 minutes prior.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed food scale soiled with food debris near storage room, mixer blade near ovens soiled.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed spray bottle of sanitizer with no label.
