Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw fish and shrimp behind cooked bacon at Make Table across cookline Reach in Freezer. Operator placed cooked bacon bits behind raw food items. **Corrected On-Site** **Repeat Violation**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw chicken 55F and cooked sausage 54F at Reach in Cooler next to grill at cookline. Operator placed in Display Cooler Unit to cool to 41F or below. **Corrective Action Taken**
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed multiple milk gallons not date marked at Display Cooler Unit. Operator date marked the food item. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener holder.
16-37-1    Intermediate - No chlorine/quaternary chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses.
