Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed kitchen ceiling tile soiled.
14-11-5    Basic - Equipment in poor repair. Observed reach in cooler cover lid broken across the cook line. Observed reach in cooler not working. Digital thermometer reads 52F. **Repeat Violation**
36-17-5    Basic - Floor tiles missing and/or in disrepair. Observed missing and cracked tiles in the kitchen.
33-11-4    Basic - Missing drain plug at dumpster.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign at hand wash sink near reach in cooler in kitchen.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed fan near three compartment sink soiled. Observed hood system filters soiled.
33-16-4    Basic - Open dumpster lid.
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface. Observed reach in cooler near cook line with rusted shelves.
08B-12-5    Basic - Stored food not covered. Observed plastic bin of raw pork not covered inside walk in cooler. **Repeat Violation**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed kitchen walls near cook line soiled.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed three containers with salt and seasonings not labeled in back prep area.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed bin of raw pork over cooked lafe moros inside walk in cooler. Operator moved products in correct order. **Corrected On-Site**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed chimichurri (76F - Cold Holding) near front window counter in kitchen. As per operator, product was taken out of cooler about 4 hours ago. Operator discarded.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed liquid eggs (56F - Cold Holding); lettuce (55F - Cold Holding); cut tomatoes (46F - Cold Holding); pico de gallo (52F - Cold Holding) at the reach in cooler across cook line. As per operator, products have been there for about two hours. Operator moved products to another reach in cooler. Observed chimichurri (76F - Cold Holding) near front service window in kitchen. As per operator, product was taken out of cooler about four hours ago. Operator discarded chimichurri. **Corrective Action Taken** **Repeat Violation**
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided form to operator. **Corrective Action Taken**
