Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine with accumulation of old like substance. Located by the walk in cooler.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling and vents soiled by the ceviche station.
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting.observed metal pans wet nesting at dry storage. **Repeat Violation**
14-11-5    Basic - Equipment in poor repair. Observed reach in cooler gaskets torn located in front of the grill. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Observed a plastic container with oil and another with vintage r stored on the floor. Operator removed them. **Corrected On-Site** **Repeat Violation**
21-04-4    Basic - In-use wet wiping cloth/towel used under cutting board. Observed a wet towel under yellow cutting board . Operator removed it. **Corrected On-Site** **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets and walk in cooler and walk in freezer gaskets soiled . **Repeat Violation**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed at the ceviche area. **Repeat Violation**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw shell eggs stored above raw beef. Operator placed raw eggs at the bottom of the reach in cooler. **Corrected On-Site** **Repeat Violation**
12A-02-4    High Priority - Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Observed Server handled soiled dishes and returned to dining area without washing hands. Employees were coach to wash hands.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener soiled. **Repeat Violation**
