Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed soiled ceiling tiles in kitchen and men's and women's bathrooms.
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Observed water dripping down from containers stacked atop dish rack.
14-11-5    Basic - Equipment in poor repair. The gasket seals on the following equipment are torn: Walk in cooler next to storage rack, reach in coolers and stand up refrigerator/freezer next to cooking station.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed soiled floors throughout kitchen and prep areas.
08B-38-4    Basic - Food stored on floor. Observed a container of lard stored on floor beneath prep table across from cook station. **Repeat Violation**
36-24-5    Basic - Hole in or other damage to wall. Observed holes in wall in men's bathroom.
51-18-6    Basic - No copy of latest inspection report available. Person in charge unable to provide last inspection.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no sign at hand wash sink next to cooking station .
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on the following equipment soiled: walk in cooler near storage rack; reach in coolers next to prep line stand up refrigerators near cooking station.
33-16-4    Basic - Open dumpster lid. Observed lid open on dumpster.
35B-03-4    Basic - Outer openings not protected with self-closing doors. Observed rear exterior door near dish rack unable to self close as closing mechanism is inoperable.
33-31-5    Basic - Unnecessary items/unused equipment on the premises. Establishment has multiple pieces of unnecessary equipment and miscellaneous materials spread throughout the premises.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed soiled walls throughout kitchen and prep areas.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 flying insect above white stand up kitchen refrigerator and 1 in men's bathroom.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw beef and chicken stored over ready to eat foods in white stand up freezer next to cook station. Foods moved and stored correctly. **Corrected On-Site**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed cleaning chemicals improperly stored near food boxes beneath prep table and beneath front counter. Chemicals moved and stored correctly. **Corrected On-Site**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed hand wash sink across from cook station with wiping cloth and pan inside. Person in charge charged removed items from sink. **Corrected On-Site**
53A-02-7    Intermediate - Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location. Person in charge unable to proof of one of establishment managers. **Repeat Violation** **Admin Complaint**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink in men's and women's bathroom and unable to dispense towels from hand wash sink next to cooking station. **Repeat Violation**
