Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

29-02-4    Basic - Toilet leaking. Toilet leaking large amounts of sewage onto the floor inside employee restroom in kitchen near cookline. **Warning**
36-50-4    Basic - Unclean building components, attachments or fixtures. Wall soiled in storage area above vegetable cooler. Floor soiled below equipment throughout kitchen. **Repeat Violation**
12A-29-4    High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee at cookline touched shirt, face, personal phone with gloved hands, then continued to touch bags of food and grill steaks at the cookline. Manager instructed employee to wash hands during this inspection. **Corrective Action Taken**
08B-27-4    High Priority - Food placed in soiled container/equipment. Unwrapped ham stored directly on visibly soiled shelf inside reach-in cooler at cookline. Cooked hamburger patty stored directly on visibly soiled shelf inside reach-in cooler at cookline. Manager voluntarily discarded both during this inspection. **Corrective Action Taken**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Pan containing raw beef stored on shelf above pan containing cooked sausage inside reach-in cooler at cookline. Manager moved raw beef to the bottom shelf during this inspection. Raw shell egg stored on shelf above open box containing sticks of butter inside reach-in cooler in storage area. 28-26-5    High Priority - Sewage/wastewater backing up through floor drains. Toilet leaking large amount of sewage onto the floor inside employee restroom in kitchen near cookline. **Warning**
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Employee at cookline washed gloved hands in handwash then continued to prepare food at cookline. Inspector explained proper handwashing and glove use to employees during this inspection. **Corrective Action Taken**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Spray bottle containing bleach and aerosol air freshener stored on shelf above cans of beans and box of dry pasta in rear storage room. 02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Unwrapped ham missing date on shelf inside reach-in cooler at cookline. Manager stated the ham was opened two days prior and voluntarily discarded the ham. **Corrective Action Taken** **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Can opener blade rusted on prep table in storage area. Interior shelving of reach-in coolers at cookline soiled with debris. **Repeat Violation**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Handwash sink at cookline blocked by large pan and bucket. Manager stated the employees utilize the sinks at the kitchen entrance and in prep room. Manager had employees unblock and set up handwash sink at cookline during this inspection. **Corrected On-Site**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Towels missing at handwash sink at kitchen entrance. Manager replenished towels during this inspection. **Corrected On-Site**

Pan containing cooked sausage missing date on shelf inside reach-in cooler at cookline. Manager stated the sausage was cooked two days prior and added proper date mark.
