Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On counter of prep table on make line in kitchen, employee phone and car keys. Operator removed from prep table. **Corrected On-Site** **Repeat Violation**
36-73-4    Basic - Floor soiled/has accumulation of debris. Floor under wok station soiled with build up and grease.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. At wok station in kitchen, tongs sitting on handle of hand cart. Operator removed tongs. **Corrected On-Site**
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. In sugar storage container in dry storage room had handle in direct contact with product. Operator lifted handle. **Corrected On-Site**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Side of fryer has grease build up. Gaskets of reach in cooler and reach in freezer at bar area soiled. **Repeat Violation**
36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. Strong objectionable odor emitting from customer men's restroom.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Wet wash cloth sitting on cutting board on prep table and at wok station not stored in sanitizer solution between uses. Operator removed wiping cloths. **Corrected On-Site**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Stored container of sugar in dry storage room was not labeled. Operator added label. **Corrected On-Site**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed one flying insect in kitchen area.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. In reach in freezer located in old sushi bar area, raw chicken stored over cooked shrimp. Operator removed raw chicken from above. **Corrected On-Site**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash station at bar area. Operator supplied paper towels. **Corrected On-Site**
53B-05-5    Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Two employee working had expired training from 2018. Operator stated he will have them update training. **Repeat Violation**
