Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Several packages of fish in reach in cooler thawed with instructions to remove product before thawing . Cook stated fish placed in cooler yesterday and began opening packages. **Corrective Action Taken**
35A-03-4    Basic - Dead roaches on premises. One dead roaches on floor next to ice machine in kitchen area. One dead roach on outlet in wall near prep table containing waffle iron. Two dead roaches on floor in ware wash area under drain boards. **Warning**
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Opened water bottle on prep table in kitchen. Moved during this inspection. **Corrected On-Site** **Repeat Violation**
08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. Employee food on shelf over sauces in reach in cooler in server station. Moved during this inspection. **Corrected On-Site**
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employee wearing bracelet while cooking on cookline. **Repeat Violation**
36-24-5    Basic - Hole in or other damage to wall. Outlet cover missing on partition wall near dish machine creating hole. Hole on side of outlet cover on cookline under prep table containing waffle iron.
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of microwave soiled with food debris.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Fan in kitchen soiled with dust. Shelves near office soiled with dust. Hood filters soiled with grease.
36-50-4    Basic - Unclean building components, attachments or fixtures. Floor under equipment on cookline soiled with grease and food debris. Wall behind fryers soiled with grease.
12A-12-4    High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. Female cook placed burger on grill with gloved hand, removed glove then poured fries into fryer without first washing hands.
12A-28-4    High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Female employee on cookline touched apron than began breading fish without washing hands.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Two live roaches on floor on cookline under hand sink and prep table containing waffle iron. One live roach under base of hand sink on cookline. Two live roaches on partition wall between prep table near mop sink and drain boards for dish machine. **Warning**
35A-23-4    High Priority - Roach excrement and/or droppings present. Black pepper like marks on wall under prep table on cookline containing waffle iron. **Warning**
03F-02-5    High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Potatoes being held on time does not have time of 9 am documented in written plan or else where. Manager stated the potatoes are taken out at 9 am and discarded every day at 1 pm. Documentation provided onsite were updated to include time during this inspection.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Employee prescription medication stored on shelf over spinach in walk in cooler. Two grey labeled degreaser chemical bottles on shelf with condiments in server station. All were moved during this inspection. **Corrected On-Site**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device and/ or soap provided at handwash sink. One sink in server station towel dispenser not working. Sink in ware wash area with no towels. Sink in server station with no soap. Manager filled all. **Corrected On-Site**
27-16-4    Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Sink in server station has hot water at 70F. Sink on cookline has hot water turned off underneath. Manager turned on hot water at sink on cookline. **Corrective Action Taken**
