A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.

Home delivery kitchen - cooked spinach thawing at room temperature. Operator moved item to walk in to quick chill. **Corrected On-Site** **Warning**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
1 small flying insect on prep table in main preparation area in kitchen.
1 small flying insect flying at entrance to roll up freezer door in main preparation kitchen (United Home Care). Did not land. **Repeat Violation** **Warning**
High Priority - Rodent activity present as evidenced by rodent droppings found.
Approximately 4 rodent droppings next to and behind blast chiller in main kitchen prep room.
2 rodent droppings behind ice machine next to door to walk in cooler in main prep kitchen. Ice machine not operating.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
Home delivery kitchen - bags of cooked spinach (60F). Observed item in crates at no temperature control. Per operator item at room temperature for less than 4 hours. High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb.

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment.
