Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. -Observed accumulation of black mold like substance buildup on AC vents and ceiling tiles throughout establishment. **Warning**
35A-03-4    Basic - Dead roaches on premises. -Observed 2 dead roaches in dry storage area underneath food storage shelves. **Warning**
14-11-5    Basic - Equipment in poor repair. Observed walk in cooler ambient temperature at 50 degrees Fahrenheit **Warning**
08B-38-4    Basic - Food stored on floor. -Observed various food items stored on walk-in freezer floor. **Warning**
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. -Observed heavy ice buildup inside walk in freezer. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. -Observed approximately 20 live flying insects in food preparation areas landing on various food items and takeout containers. -Observed approximately 10 live flying insects in dishwashing areas landing on clean food containers . -Observed approximately 10 live flying insects in service area next ice machine. 08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. -Observed raw shell eggs stored over stuffing and dressings in walk-in cooler. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. -Observed 6 live roaches in kitchen next to microwaves and Hobart mixer. Observed 2 live roaches in preparation area between food prep sink and food storage shelves. -Observed 1 live roach behind ice machine **Warning**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed raw chicken cold held at 49-52°F. Per operator, all chicken placed in walk-in cooler from yesterday. Chicken didn't removed from unit since. See stop sale. **Repeat Violation** **Warning**
