Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled and kitchen hood with accumulated grease. Observed murky water from the fish tank with foul odor draining into a dirty container. **Warning**
36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. By the fish tank, by the kitchen exit door, inside the kitchen and in the the dining area. **Repeat Violation** **Warning**

29-03-4    Basic - Water draining onto floor surface. Observed a pipe draining water from the refrigeration unit through the wall of the walk in freezer into a bucket by the hand washing sink. **Repeat Violation** **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed on the cook line container of salt, sugar, soy sauce, and oil with no name label. **Warning**
36-10-4    Basic - Worn, torn and/or soiled floors/carpeting. Observed floor under the stove soiled, under the dishwasher soiled, under the prep tables soiled. Observed floor of walk in cooler soiled. **Warning**
03D-02-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked chicken (56F - Cooling) in the reach in cooler stored overnight. Operator discarded items. **Corrective Action Taken** **Warning**
01B-36-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within two hours and from 135 degrees Fahrenheit to 41 degrees Fahrenheit within six hours. See stop sale. Observed cooked chicken (56F - Cooling) in the reach in cooler stored overnight. Operator discarded items. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 fly landing on foam plates in the kitchen. **Warning**
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed license expired on 10/01/23. Advised operator license is expired, encouraged operator to pay the license during the inspection. **Corrected On-Site** **Warning**
08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed raw pork stored over raw fish in the walk in cooler. Discussed and provided hand out to operator. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed at the front counter: approximately 15+ rodent droppings on the lower shelves, then approximately 6 rodent droppings underneath the fish tank, 4 rodent droppings inside a bucket underneath the large fish tank, kitchen area: 5 rodent droppings on the floor underneath the shelves with the packets of sauces, 4 rodent droppings in between both reach in coolers, 1 rodent dropping on the floor up in front of the walk in freezer, approximately 15 rodent droppings underneath the dishwasher, dry storage area: approximately 20+ rodent droppings on the floor underneath containers with rice and flour, 3 rodent droppings on the floor underneath the electrical box, 4 rodent droppings on the floors by the entrance to the dry storage room, Women restroom: 2 rodent droppings on the floor inside the handicap stall and 1 rodent dropping on the floor underneath the hand washing sink and men's restroom: 1 rodent dropping on the floor by the hand washing sink. **Repeat Violation** **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed fresh garlic oil (67F - Cold Holding) in the cook line, as per operator taken out 3 hours before inspection. Placed back inside the reach in cooler. **Corrective Action Taken** **Warning**
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Emailed form to operator. **Corrective Action Taken** **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener soiled in the kitchen. **Warning**
11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Emailed form to operator. **Warning**
35A-01-4    Intermediate - Nonservice animals in the food establishment or on premises. Observed a live cat in the dining area resting on the floor, the cat bed on a chair in the kitchen by the office table and cat food bag stored in the dry storage area. Inspector asked operator to remove live cat from the establishment. Observed the operator remove the cat inside the cage. Observed cat carrying case by the mop sink outside, exit door was left open, cat walked inside the kitchen and operator removed cat again. Advised the operator the cat can not be on the premises, if the cat is observed in future inspection, it will result in an an immediate AC. **Warning**
27-11-4    Intermediate - Observed no running water at mop sink outside by the dumpster. **Warning**
53B-05-5    Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed Qiu Mei Lin, Yong Bo Liu and Jing Chen all expired certificate on 10/5/23. **Warning**
