A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Sides of fryers and flattop at cook line. Gaskets on multiple reach-in cooler/freezers throughout establishment. **Repeat Violation**
25-05-4    Basic - Single-service articles improperly stored. At dish area, paper towels stored on floor. At men's restrooms storage closet, toilet paper stored on floor.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. At triple sinks, hamburger thawing in standing water. Employee moved to running water. **Corrected On-Site**
32-23-4    Basic - Toilet/urinal not flushing/functioning properly. Urinal in men's restroom out of order. **Repeat Violation**

51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. 53B-14-5    Intermediate - Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. One employee certificate expired.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. At handwash sink on cook line, spray bottle identified as window cleaner not labeled.
27-02-4    Intermediate - Well water testing report/documentation is not available upon request. No bacteriological report available. **Repeat Violation**
