Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Large hole in ceiling in kitchen near back door. Large section of drywall missing from ceiling over ice machine in storage area in back of kitchen.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee keys and electronic smoking device stored on wire shelf above cabbage on prep table in back of kitchen near three-compartment sink. Employee removed keys and smoking device from shelf during this inspection. **Corrected On-Site** **Repeat Violation**
36-24-5    Basic - Hole in or other damage to wall. Hole in wall near back door of kitchen. Lower portion of walls damaged in storage area near ice machine.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Heavy ice buildup on shelving inside reach-in freezer at cookline. **Repeat Violation**
14-47-4    Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Lower portion of prep table near three-compartment sink rusted.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach-in freezer near cookline soiled with food debris. **Repeat Violation**
36-50-4    Basic - Unclean building components, attachments or fixtures. Floor below cooking equipment in kitchen soiled with grease and/or food debris. Floor inside walk-in freezer soiled with food debris. Ceiling near hood at cookline soiled with dust.
03F-02-5    High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Raw shell eggs in pan at cookline without time mark. Manager stated the eggs have been in-use for approximately one hour and added proper time mark during this inspection. **Corrected On-Site**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Backflow prevention device missing at spigot on wall below dish machine. **Repeat Violation** **Admin Complaint**
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Sausage stored inside zip bag missing date mark on shelf in reach-in cooler in back of kitchen. Manager stated the sausages were opened on 2/18 and added proper date mark during this inspection. Pan of mashed potatoes missing date mark on shelf inside walk-in cooler. Manager stated the potatoes were prepared on 2/18 and added proper date mark during this inspection. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Can opener blade soiled with black substance. Interior walls of ice machine soiled with brown substance. Cutting board stained black on prep table near toasters. **Repeat Violation**
31B-02-4    Intermediate - No soap and/or paper towels/mechanical hand drying device provided at handwash sink. Towels missing from handwash sink at right-side of cookline. Soap missing from handwash sink at left end of cookline. Manager replenished both during this inspection. **Corrected On-Site**
