Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-22-4    Basic - Floor area(s) covered with standing water in multiple areas throughout the back and middle of the house. **Warning**
36-17-5    Basic - Floor tiles missing and/or in disrepair. Missing floor tiles and grout in multiple areas of the middle and back of the house. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Heavy black buildup on exterior of cold tables on the cook line. Old labels observed on plastic pans stored as clean in the dish area. **Warning**
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed chicken being thawed in a slotted pan under running water and not submerged. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Walls soiled with encrusted food debris in multiple areas throughout the middle and back of the house. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 4 small flies in the bar area, specifically around the beer taps, 4 small flies in the server station area and 3 small flies in the dish area near the hand wash sink and 5 in dry storage and one by the back door. **Warning**
03G-50-1    Intermediate - Operator is doing a special process, conducting reduced oxygen packaging, cooking time/temperature control for safety food to a time and temperature combination lesser than those in the Food Code or operating a molluscan shellfish tank for human consumption without a HACCP plan approved by the Division of Hotels and Restaurants. Conducting cook/chill on soups and sauces and holding more than 48 hours with no HACCP plan. Discussed with Operator the need to open cook/chill bags within 48 hours or submit a proposed HACCP plan for approval. **Warning**
16-54-4    Intermediate - Rinse solution in spray type hot water sanitizing dishmachine not reaching proper temperature according to manufacturer's specifications. Gauge for rinse cycle indicated 74F. Discussed specifications with Operator and recommended calling for service. **Warning**
