Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. At the end of the cook line, near walk-in freezer, observed two dead roaches under shelves. **Warning**
08B-38-4    Basic - Food stored on floor. At the cook line, under wok, observed oil jug on floor, in walk-in cooler observed multiple cases of raw food on floor. **Repeat Violation**
10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. At the cook line, observed knife stored between reach-in coolers. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of cooking equipment, tables and under equipment soiled. **Repeat Violation**
12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Opened employee drinks inside reach-in cooler at the cook line and walk-in cooler.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior of reach-in coolers and shelves heavily soiled.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. At the prep area, observed raw pork and cooked pork thawed at room temperature, both items were at 43F. **Repeat Violation**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Walk-in cooler shelves. **Repeat Violation**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. At the dry storage area, containers with sugar, salt and flour not labeled. **Repeat Violation**
41-07-4    High Priority - Container of medicine improperly stored. At the front reach-in cooler, observed medicine stored inside reach-in cooler above customers food, also at the cook line, observed box with multiple medicine stored above prep table in use.
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Female employee touching multiple dirty surfaces and taking orders, started working with food and no hand washed occurred. **Repeat Violation** **Admin Complaint**
12A-27-4    High Priority - Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. At the cook line, male employee cracked a raw shell eggs and immediately after handle a cleaned plastic to go container and served soup and cooked egg (no observable cross contamination observed) Inspector addressed all handwash violations with person in Charge. **Warning**
12A-12-4    High Priority - Employee switched from working with raw food to ready-to-eat food without washing hands. At the prep area, observed Male employee working with raw frozen chicken switched to work with cooked food (chicken) and handled cleaned containers. **Warning**
09-01-4    High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. At the prep area, observed male employee cutting Chinese cabbage with no gloves, Inspector instructed to washed his hands and use gloves, employee started wearing gloves no hand washed occurred. **Warning**
22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. At the prep area, observed male employee picking knife from floor and placing on magnet for later use, Inspector instructed to perform sanitization process, employee rinsed knife with water and placed back to magnet, no soap applied No sanitization occurred.
08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. At the prep area, observed raw scallops bag stored above vegetables.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. At the cook line: observed 12 live roaches around electric outlet behind reach-in cooler. One live roach on reach-in cooler lid at the cook line, Manager killed. One live roach on wall behind shelves. One live roach under handwash sink at the end of the cook line. **Warning**
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. 05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. No probe thermometer available.
53B-02-5    Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. 11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. For three employees.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. In reach-in cooler and walk-in cooler, multiple food items no date marked, per Manager most of the food were cooked or prepared with in the last 3 days. **Repeat Violation**
