Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Observed 30 dead roaches on the floor underneath reach in cooler, 3 compartment sink, and under dry food storage shelving . Also 5 dead roaches by the hand sink at kitchen entrance from dining area.
36-22-4    Basic - Floor area(s) covered with standing water. Observed standing water on the floor around the reach in cooler. **Repeat Violation**
36-11-4    Basic - Floors not maintained smooth and durable. Floor not maintained smooth or durable. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Observed oil container stored on floor at cook line. Also observed raw chicken and beef on walk in freezer floor. **Repeat Violation**
36-24-5    Basic - Hole in or other damage to wall. Observed in different places in the establishment.
14-42-4    Basic - Hood filter missing from automatic fire suppression/exhaust system. Observed missing filter at the kitchen hood.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed at walk in freezer.
10-12-5    Basic - In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored on the top of the ice machine. Owner moved it immediately. **Corrected On-Site**
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed oven interior soiled with old food debris. **Repeat Violation**
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed no ambient thermometer at reach in cooler.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed exterior of equipments soiled with old food debris. **Repeat Violation**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed at walk in cooler. **Repeat Violation**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Observed mop not stored to dry. **Repeat Violation**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken stored over egg rolls at reach in cooler. Operator rearranged products. **Corrected On-Site**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 4 live roaches crawling on the kitchen floor behind reach in cooler, 2 live roaches crawling on the floor in the kitchen area by the walk-in cooler door, 4 live roaches crawling on the floor of the dry food storage area, 1 live roach crawling on the floor by the kitchen exit door.
41-25-4    High Priority - Tracking powder pesticide used inside establishment. Observed tracking powder in use all throughout establishment.
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed at hose connected inside kitchen.
53B-02-5    Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233 ( Samuel and Mauricio ) .
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked chicken and beef no date marked at walk in cooler. As per operator from 2 days prior.
