A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises.
Observed one dead roach at entrance of walk in freezer.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils.
Observed employee drink stored of table near ware washing.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust.
Observed reach in cooler door gaskets on cook line soiled.
Observed walk in cooler door gasket soiled.
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Observed 4 flying insects near ware washing area. **Repeat Violation** **Admin Complaint**
High Priority - Nonfood-grade bags used in direct contact with food.
Observed frozen beef stored in non food grade to go bags stored in walk in freezer.
Employee stated he will place in zip lock bags. **Corrective Action Taken**
Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Observed no certified food manager available with 4 employees engaging in food preparation. **Repeat Violation**
