Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. On prep table in kitchen, employee sunglasses and employee purse. Both were removed. **Corrected On-Site**
14-11-5    Basic - Equipment in poor repair. Door of walk in freezer is breaking apart, with insulation starting to show. **Repeat Violation**
36-17-5    Basic - Floor tiles missing and/or in disrepair. Multiple floor tiles missing/disrepair under and around fryers. **Repeat Violation**
36-73-4    Basic - Floor, walls and or ceilings soiled/has accumulation of debris. Floor under and behind fryers grease build up. Ceiling vents throughout kitchen soiled with dust. Ceiling tiles above fryers and flat top have grease build up.
08B-38-4    Basic - Food stored on floor. Cooking oil on ground next to fryer in kitchen. Three cases of chicken on ground in walk in freezer.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. At walk in freezer, Ice buildup up on ground and around door.
10-08-5    Basic - Ice scoop handle in contact with ice. Scoop handle in direct contact with ice.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets of reach in cooler under flip top on make line is soiled with food debris. Gasket of walk in cooler is soiled with food debris.
08B-12-5    Basic - Stored food not covered. Two tea urns at server station uncovered. **Repeat Violation**
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Raw beef being thawed in sink with standing water. Employee turned on running water. **Corrected On-Site**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Stored flour container on storage rack by back door not labeled.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed one flying insect in kitchen.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. In walk in cooler, raw chicken stored over cooked chicken. Employee moved to correct storage levels. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
41-10-4    High Priority - Toxic substance/chemical improperly stored. At food pass through window in kitchen, windex cleaner on ledge of window. Employee removed windex. At chemical storage rack by ice machine and manager office, hand soap stored over paper towels. Employee move to correct storage levels. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board on make line has mold-like substance. **Repeat Violation**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Hand wash station at bar has two dirty utensils in sink.
53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. A list of accredited food manager certification examination providers can be found at http://www.myfloridalicense.com/DBPR/hotels-restaurants/food-lodging/food-manager/ Upon arrival, no CFM manager on site. Manager arrived half way through inspection.
53B-14-5    Intermediate - Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Two employees training expired.
