Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Two dead roaches, one in microwave screen One dead roach in cabinet under handwashing sink. Two dead roaches under back hand sink in kitchen.
35A-20-4    Basic - Dead rodent present. Dead rodent in water heater room, Operator cleaned at time of inspection. Discussed sanitizing. **Corrective Action Taken**
12B-02-4    Basic - Employee eating in a food preparation or other restricted area. Employee eating In dish washing area. Operator had employee moved at time of inspection. **Corrective Action Taken**

35B-02-4    Basic - Insect control device installed over food preparation area. Bug zapper installed over prep sink, discussed with operator. Operator removed at time of inspection. **Repeat Violation**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Hand washing sign at side hand sink faded and no longer legible.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gasket on reach in cooler, and walk in cooler soiled. **Repeat Violation**
24-11-4    Basic - Salad bar/buffet plates/bowls not properly protected or inverted to prevent contamination. Bowls and plates at salad bar not inverted.
36-30-4    Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Wall not smooth and easily cleanable , in dry storage shed next to mop sink. Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. 21-38-4    Basic - Wiping cloth sanitizing solution stored on the floor. Sanitizer stored on floor, operator moved to approved location at time of inspection. **Corrected On-Site**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. 3 - 10 pound cans sliced peaches dented.
12A-17-4    High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. Operator washed hands approximately 5-10 seconds, discussed with operator.
09-01-4    High Priority - Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee touch ready to eat watermelon while prepping, operator hand employee rewash hands and put on gloves at time of inspection. **Corrective Action Taken**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 8 live flies under 3 compartment sink in dish area.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 5 rodent droppings in water heater room 7 rodent droppings under dry storage shelf. 10 rodent droppings under dry storage pallet 22 rodent droppings under dry storage shelf near back storage area. 1 rodent dropping on top of food storage shelf. 12 rodent droppings observed on top of dishwasher. Discussed with operator.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cut Tomato (50F - Cold Holding) operator voluntarily discarded at time of inspection.
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink in back missing vacuum breaker.
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Observed employee wash hands in prep sink, discussed with operator
