Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Observed one dead roach in small garbage can behind unused sushi bar. Person in charge discarded. **Corrected On-Site** **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed; floors in back dry storage area under prep table soiled with grease build up. Floors under fryers soiled with grease build up.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed tongs hanging from cart across from fryer station with possible exposure to contact with employee clothes. Person in charge removed tongs. **Corrected On-Site** **Repeat Violation**
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed rice scoop for white rice at steam table in standing water with temperature of 85F. Person in charge voluntarily discarded. **Corrected On-Site**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed; soiled gaskets on reach in cooler under flip top cooler across from wok station. **Repeat Violation**
08B-12-5    Basic - Stored food not covered. Observed multiple food items in reach in cooler under flip top cooler across from wok station with no cover. Person in charge began covering items. **Corrective Action Taken**
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed white powdery substance on prep shelf across from three compartment sink person in charge stated was boric acid. **Corrective Action Taken**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed 50+ rodent droppings in cabinet located in unused area of service station in dining room. Unused service station is separate from kitchen. Person in charge stated that area has not been in use for months. Person in charge began cleaning cabinet. **Corrective Action Taken** **Warning**
53B-05-5    Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed all food handler certifications expired. **Repeat Violation** **Warning**
