Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Observed accumulation ofold food residue on walk in freezer floor. **Warning**
08B-38-4    Basic - Food stored on floor. -Observed bag with flour stored on dry storage floor. -Observed cases with various food items stored on walk in cooler and freezer floor. **Repeat Violation** **Admin Complaint**
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed container storage in bag with flour. **Warning**
35B-02-4    Basic - Insect control device installed over food preparation area. -Observed bug zapper installed in wait station above clean cups and take out containers. **Repeat Violation** **Admin Complaint**

22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine sanitizer solution at dishwashing machine at 00 ppm. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. -Observed approximately 20 live flying insects in wait station next to steam table. -Observed approximately 10 live flying insects landing on soda nozzle and clean cutting board in bar. **Repeat Violation** **Admin Complaint**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. -Observed raw pooled eggs stored over cut tomatoes at flip top cooler. Operator removed and relocate raw eggs. **Corrected On-Site** **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. -Observed 6 rodent droppings on top of dishwashing machine. -Observed approximately 10 rodent droppings underneath dishwashing machine. -Observed 5 rodent droppings behind flip top coolers at cookline. -Observed 4 rodent droppings underneath food preparation table at wait station. **Warning**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed roast beef cold held at 48-52F in walk in cooler. Per operator, beef cooked and placed in unit from yesterday. Roast beef did not remove from unit since. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. -Observed tuna salad cold held in flip top cooler at 45F. Per operator, item transfer from walk in cooler to flip top cooler 2 hours prior to the inspection. Operator place tuna salad on ice bath. -Observed roast beef cold held at 48-52F in walk in cooler. Per operator, beef cooked and placed in unit from yesterday. Roast beef did not remove from unit since. See stop sale. **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. -Observed accumulation of black mold like substance buildup on can opener blade. **Repeat Violation** **Admin Complaint**
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. **Repeat Violation** **Admin Complaint**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. -Observed an unlabeled spray bottle with degreaser at dishwashing area. **Repeat Violation** **Admin Complaint**
