Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-67-4    Basic - Establishments only bathroom- Exhaust fan inoperable in bathroom with no other form of ventilation.
36-22-4    Basic - Floor area(s) covered with standing water. Standing water on floor in front of flip top cooler in kitchen.
36-17-5    Basic - Floor tiles missing and/or in disrepair. Missing floor tiles in front of flip top cooler in kitchen
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood vent in kitchen soiled with dust at cookline
36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. Observed objectionable odor in hallway with bathroom leading storage room with triple sink
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Standing water in RIC in kitchen.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Presence of insects, rodents, or other pests. 1. Approximately 5 small flying insects in hallway leading to dry storage containing triple sink 2. Approximately 5 small flying insects in kitchen- between hand wash sink and flip top cooler at cookline.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 1.Approximately 9 rodent droppings in exterior storage unit. Storage unit is separate from kitchen and contains a reach in cooler, reach in freezer, and dry storage items- pizza boxes 2.Approximately 4 rodent droppings in hallway leading to triple sink under dunnage wracks containing preformed pizza boxes. Operator began cleaning and sanitizing the area.
