Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-06-4    Basic - Cardboard used on floor as anti-slip measure not replaced every day or when heavily soiled, whichever comes first. Observed throughout the kitchen area.
14-45-4    Basic - Cardboard used to line nonfood-contact shelves. Observed cardboard being used for lining throughout the kitchen area. **Repeat Violation**
36-36-4    Basic - Ceiling tile missing. Observed ceiling tile missing on top oven .
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents soiled with black substance in kitchen area and front counter area.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed grooved cutting board at preparation table across the oven , manager discarded during inspection. **Corrected On-Site**
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed employee handling food without a hair restraint, manager provided hair net during inspection. **Corrected On-Site**
14-11-5    Basic - Equipment in poor repair. Observed reach in freezer across oven and walk in cooler gaskets torn.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed floor soiled with grease under equipment in kitchen area.

36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed wall soiled with grease and food debris throughout the kitchen area.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry.
08A-14-5    High Priority - Raw animal food not separated from ready-to-eat food during preparation. Observed marinated raw shrimp on top cooked pork inside reach in cooler , manager removed and stored properly. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting board soiled with food debris non top preparation table across the oven. Manager removed and washed properly during inspection. **Corrected On-Site**
22-31-4    Intermediate - Non-pitting surface rust on food-contact equipment. Observed shelves under preparation tables surfaces rusted throughout the kitchen area.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed mixed rice not date marked inside reach in cooler , as per manager less than 24 hours. Manager labeled product during inspection. **Corrected On-Site**
