Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed several mixing bowls and pots not inverted on drying racks at ware-washing area. **Warning**
06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed in the preparation cooler across from the stove at cook line raw Snapper not removed from ROP package before thawing, raw Snapper no longer frozen. Employee discarded fish on site. Observed raw tuna not removed from ROP package before thawing, tuna was no longer frozen. Operator discarded tuna on site. **Corrected On-Site** **Warning**

10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed in use utensils in standing water at 81F, stored on top of under-counter cooler at cook line. **Warning**
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of oven soiled, oven located at cook line. Observed interior of both microwave soiled, stored on a small prep table at cold station. **Warning**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed no hand wash sign available for employees at hand wash sink located at the bar area by the main kitchen. **Warning**

Observe gasket soiled on refrigerated drawer underneath the grill at cook line. Observed ice scoop stored on top soiled ice machine surface. **Warning**
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed old labels in clean container stored on drying racks at warehouse-washing area. **Warning**

38-04-4    Basic - Ten (10) foot-candles of light at least 30 inches from floor not provided in walk-in cooler. **Warning**
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed raw pork leg thawing in standing water on a prep table located by the ware-washing area. **Warning**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry, at mop sink station. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed salt and pepper containers, squeeze bottles with mojo not labeled. Employee labeled items on site. **Corrected On-Site** **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately around 6 small flying insects flying around the mop sink area not touching or landing on food or food contact surfaces. **Warning**

08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw tuna stored over cheese in the reach in cooler at cold station. **Warning**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed in the walk in cooler raw chicken stored over raw steak. **Warning**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed in the preparation cooler across from the stove at cook line raw Snapper not removed from ROP package before thawing, raw Snapper no longer frozen. Employee discarded fish on site. Observed raw tuna not removed from ROP package before thawing, tuna was no longer frozen. Operator discarded tuna on site. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sautéed onions (76F - Cold Holding), on the grill at cook line, as per cook out of temperature for less than 2 hours. 03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At bar#2 in a hot holding unit observed beef empanadas at 102f, as per operator held in unit for less than 4 hours. 53A-10-4    Intermediate - Certified food manager unable to answer basic Food Code questions pertaining to safe operation of establishment. Certified manager was unable to answer the minimum temperature for hot holding, stipulated by the food code. 53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.
