Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed at reach in cooler across from stove. Employee was coached to remove fish before putting it to thaw.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed at white cutting board at back of kitchen. **Repeat Violation**
35A-03-4    Basic - Dead roaches on premises. Observed 1 dead roach on wall above three compartment sink in kitchen area. Observed 2 dead roaches on preparation table in kitchen area. Observed 5 dead roaches on preparation table above clean dishes in kitchen area. Also observed 2 dead roaches on floor in dry storage area.
12B-02-4    Basic - Employee drinking in a food preparation or other restricted area. Observed multiple employees cups stored in preparation table at front counter and also in preparation table in kitchen area.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee sunglasses stored above preparation table in back of kitchen. **Repeat Violation**
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed two cooks wearing watches while preparing food at front line.
13-04-4    Basic - Employee with no beard guard/restraint while engaging in food preparation. Observed cook preparing food with no beard restraint.
14-11-5    Basic - Equipment in poor repair. Observed reach in cooler gaskets torn near steam table at front counter.
51-09-4    Basic - Establishment did not report seating change that affects the license fee, Clean Indoor Air Act, sewage system approval or other related requirements. Observed 82 seats instead of 49 as per license. Operator reported that he has submitted paperwork to make change. **Repeat Violation**
36-22-4    Basic - Floor area(s) covered with standing water. Observed behind reach in freezer in back of kitchen.
36-17-5    Basic - Floor tiles missing and/or in disrepair. Observed floor in disrepair near walk in cooler near kitchen.
14-38-4    Basic - Food storage container/container lid cracked or broken. Observed raw fish in broken container in reach in cooler across from stove.
08B-38-4    Basic - Food stored on floor. Observed a box of raw chicken stored on floor of walk in cooler.
36-24-5    Basic - Hole in or other damage to wall. Observed wall without base board at entrance of dry storage. Also observed hole in wall in dry storage area.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed at reach in freezer chest in dry storage area.
10-12-5    Basic - In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored on drink machine at front counter.
10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. Observed knife stored between wall and and preparation table at front counter.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed tongs stored on oven in kitchen.
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit.
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. Observed at hand wash sink in back of establishment near bathroom
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed reach in cooler gaskets soiled at front counter near steam table. **Repeat Violation**
36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. Observed objectionable odors throughout front counter and kitchen area.
12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Observed opened Gatorade bottle stored in reach in cooler near bathroom.
25-05-4    Basic - Single-service articles improperly stored. Observed 10 boxes of to go containers and cups stored on floor of dry storage area located by the bathroom.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Observed two ice cream scoops not inverted inside metal bucket in kitchen.
29-19-4    Basic - Standing water in floor drain/floor drain draining very slowly. Observed standing sewage water at front counter next to cooking and next to dishwasher machine in kitchen area.
29-20-5    Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed slow drain at hand wash sink located near bathroom.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloths stored on preparation tables throughout kitchen. **Repeat Violation**
21-09-4    Basic - Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean. Observed soiled wet wiping cloth stored on cutting board at front line,
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed chef touch cracked eggs, phone and then put gloves on without washing hands
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed raid stored on shelf of preparation table at front line
28-13-5    High Priority - Raw sewage on ground of establishment. Observed when washing hands sewage water began overflowing from drain line onto floor of establishment at front counter. Also observed in back of kitchen near three compartment sink and dish machine.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach crawling on preparation table near mixer in kitchen area. Observed 8 live roaches crawling on preparation table in kitchen area near cooked potatoes.
01B-14-4    High Priority - Shell eggs in use or stored with cracks or broken shells. See stop sale. Observed 2 cracked shell eggs in RIC at front line. Stop sale issued.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Beef (51F - Cold Holding). As per operator, it was prepared Saturday. Observed Roast Beef (48 - Cold Holding) as per operator, it was prepared Saturday. Observed beef patty (48F -Cold Holding). Butter with garlic (64F - Cold Holding) As per operator, it was placed there the previous day.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Beef (51F - Cold Holding). As per operator, it was prepared Saturday. Observed Roast Beef (48 - Cold Holding) as per operator, it was prepared Saturday. Observed beef patty (48F -Cold Holding) and Butter with garlic (64F - Cold Holding). As per operator, it was placed there the previous day. Stop sale issued.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed sanitizer bucket stored on the floor in back of kitchen. Observed sanitizer bucket stored on preparation table at cook line.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed preparation tables in back of kitchen soiled with old food debris. Also observed microwave interior soiled at front counter. **Repeat Violation**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed ice cubes in handwash sink located near bathroom
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed at hand wash sink in back of establishment near bathroom
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed walk in cooler and walk in freezer was built at side of establishment by bathroom without plan review approval.
