Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-38-4    Basic - Food stored on floor. Observed fryer oil stored on floor underneath prep table in prep area by ovens. Observed box of corned beef brisket stored on floor inside walk in cooler. Observed container of olives stored on floor under hand sink at front counter **Repeat Violation**

14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup inside of reach in freezer next to dish rack in kitchen.
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop inside container of sugar with handle touching the sugar at back area near ice machine.
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed inside of oven soiled at prep area. **Repeat Violation**
38-01-4    Basic - Light shield damaged/in disrepair. Observed light shield in disrepair in front of ice machine in kitchen area
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed all reach in cooler gaskets soiled throughout establishment. Observed brix pumps heavily soiled with dust next co2 tanks with container of uncovered bacon stored underneath. **Repeat Violation**
22-16-4    Basic - Observed all reach-in cooler, reach in freezer, and walk in cooler interior/shelves have accumulation of soil residues throughout establishment. **Repeat Violation**

12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Observed employee water bottle stored inside of reach in cooler at prep area next to ovens
32-17-4    Basic - Self-closing device on bathroom door disconnected/broken. Observed self closing device broken on Men's bathroom door
42-03-5    Basic - Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. Observed drill stored directly on top of box of gravy mix on dry storage shelf near prep area and cables/tools stored inside bread warmer box.

14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed walk in cooler shelves rusted.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed walls soiled throughout kitchen **Repeat Violation**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Observed mop stored inside of mop bucket by mop sink
21-03-4    Basic - Wet wiping cloths stored in detergent and sanitizer mixed together. Observed cloth stored inside bucket with soap and sanitizer stored at front counter
14-06-4    Basic - Wood food-contact surface not properly sealed. Observed wood being used as bottom shelf of prep table with box of potatoes and container of onions stored on top.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed containers of sugar not labeled in back area by ice machine and container of baking powder not labeled at dry storage rack near cook line
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Observed dented can of dark red kidney beans at can rack next to ovens in prep area.
12A-13-4    High Priority - Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee walk into the kitchen from the dining room with dirty plates, then remove soiled gloves, put on clean gloves without washing hand then touched clean plates from a rack from the dish machine. Inspector coached employee to wash hands when changing tasks.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed one live flying insect in the kitchen prep area landing and walking on cutting board with cut chives.
14-31-5    High Priority - Nonfood-grade bags used in direct contact with food. Observed herbs stored in thank you bags inside of walk in cooler
14-15-4    High Priority - Nonfood-grade containers used for food storage - direct contact with food. Observed nonfood grade container used for storing sugar in back area near ice machine and non food grade container used for storing onions in kitchen under prep table.
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. Observed can of raid household pesticide stored on shelf behind ovens in prep area
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed smoked salmon stored over ready to eat pickles inside of walk in cooler. Observed container of pooled eggs stored over sauces inside of walk in cooler. Discussed with operator correct storage.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed rubbing alcohol and burn relief spray stored on dry storage shelf on top of spices. Observed chemicals stored next to box of potatoes underneath prep table near dish area
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed mold like substance on multiple containers of seafood base inside of reach in cooler in back area near ice machine
16-36-4    Intermediate - No color/concentration comparison chart available for chemical test kit used to check sanitizer. Observed test kit for chlorine without concentration chart.
22-31-4    Intermediate - Non-pitting surface rust on food-contact equipment. Observed can opener blade rusted
16-08-4    Intermediate - Observed dish machine too small to accommodate utensils or equipment. Observed multiple large pots and mixing bowls that cannot fit inside dish machine.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked meatloaf, cooked turkey breast, and cooked brisket stored on rack inside walk in cooler without date mark. As per employee was prepared 2 days before inspection. **Repeat Violation**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed spray bottle of chemicals stored underneath prep table by dish area not labeled.
