Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. On cook line, ceiling tiles have build up of grease/dust.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. In middle room of kitchen, employee cellphone on cleaned prep table. Manager removed cell phone. **Corrected On-Site** **Repeat Violation**
14-11-5    Basic - Equipment in poor repair. Multiple gaskets on cook line torn/ripped. **Repeat Violation**
36-17-5    Basic - Floor tiles missing and/or in disrepair. Multiple tiles missing under fryers on cook line. Also, tiles in disrepair in walk-in cooler in front of freezer door. Also, multiple other tiles missing through out kitchen. **Repeat Violation**
36-73-4    Basic - Floor, walls and/or ceilings soiled/has accumulation of debris. Under equipment on cook line/server line build up of food debris. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. In walk-in freezer, one case of fish and one case of meat both stored on floor. Manager is going to have employee move to shelf when they get in because she could not lift the cases. **Corrective Action Taken** **Repeat Violation**
33-19-4    Basic - Garbage on the ground and/or pad around dumpster. Around both dumpsters out back of establishment.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. In walk-in freezer, ice build up on floor and doorway/door to walk-in freezer. **Repeat Violation**
31B-04-4    Basic - No handwashing sign provided at a hand sink used by food employees. No handwash sign provided in men's restroom.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. On server line, around nozzles of soda machine have build up of mold-like substance. Also, interior of reach in cooler in bar area near Jáegermeister machine soiled with brown liquid. **Repeat Violation**
33-16-4    Basic - Open dumpster lid. On both dumpsters out back of establishment, all lids open.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. On cook line, above pass through window, metal to go bowls not inverted. Manager flipped all stacks of pans over the right way. **Corrected On-Site**
08B-12-5    Basic - Stored food not covered. On server line, tea filled urns not covered. **Repeat Violation**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. At front counter by register, two wet wiping clothes stored on counter. Manager removed them. Also, in kitchen on metal prep table one wet wiping cloth stored on table in between using. Manager removed it. **Corrected On-Site**
35A-02-7    High Priority - Live, small flying insects found. One live flying insect at bar area. One dead flying insect in reach in cooler on cook line on shell of raw shelled egg. **Repeat Violation** **Admin Complaint**
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment. In middle room of kitchen, on top shelf of rack over clean pans, a package of raid roach bombs. Manager removed from rack and placed in office. **Corrected On-Site**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. In reach in cooler on cook line, a plate of raw steak stored on shelf above open container of heavy cream. Manager removed heavy cream from reach in cooler and placed in walk-in cooler. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In reach in cooler on cook line, a plate with one raw steak stored on shelf above not covered pan of raw fish. Manager switched the products. **Corrected On-Site**
08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. In walk-in freezer, ziploc bags of raw beef stored on shelf over raw whole fish in pans covered with plastic wrap.
01B-03-5    High Priority - Stop Sale issued due to adulteration of food product. On cook line in reach in cooler, dead flying insect on shell of raw shelled egg. Manager discarded.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On cook line in reach in cooler at end of line by fryers, cut lettuce (48F - Cold Holding); cut tomatoes (48F - Cold Holding). Manager stated product was put in cooler approximately 1 hour prior at open but was filled over the fill line on the pans.
41-10-4    High Priority - Toxic substance/chemical improperly stored. On server line when entering kitchen, bottle of degreaser stored on bottom shelf with deli paper and individual salt packets. Manager removed degreaser from shelf and moved. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Handwash sink in dish area blocked by large round trash can. Manager moved trash can to clear path to handwash sink. **Corrected On-Site**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Handwash sink on server line, had bowl in it. Manager removed. Also, handwash sink in dish area, had pile of steel scrubby in it. **Corrected On-Site** **Repeat Violation**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels available at handwash sink in dish area.
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Manager was only able to provide two training certificates that are both expired at beginning of March. **Warning**
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. In middle room of kitchen, on bottom shelf of rack two spray bottles of blue chemicals not labeled.
