A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.

Basic - Dead roaches on premises.
1 dead roach under triple sink. 1 dead roach next to stove in kitchen. 1 dead roach on shelf at front counter.
Operator removed dead roaches and cleaned/ sanitized the area. **Warning**
High Priority - Rodent activity present as evidenced by rodent droppings found.
Approximately 24 rodent droppings in kitchen on shelf containing audio equipment.
Approximately 12 rodent droppings under microwave on shelf in kitchen.
Approximately 4 rodent droppings on prep table under and behind steam table in kitchen.
Approximately 2 rodent droppings on top of dish washing machine in kitchen.
Approximately 2 rodent droppings located in rear storage area by water heater. **Warning**
High Priority - Stop Sale issued due to food not being in a wholesome, sound condition.

Intermediate - Handwashing sink on front counter is double sink. Per plan review, divider must be installed and separate faucets on each sink. **Warning**
Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Poached egg on menu, reviewed menu updating with operator. **Warning**
Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. **Warning**
Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses.
