A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.

Basic - Wet wiping cloths stored in detergent and sanitizer mixed together. Soap and sanitizer mixed in sanitizer bucket. Employee removed bucket and refilled with sanitizer only. **Corrected On-Site**
High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee began working with garnishes no hand wash in bar area. Educated employee and she washed her hands. **Corrective Action Taken**
High Priority - Roach activity present as evidenced by live roaches found.

High Priority - Roach excrement and/or droppings present. Roach excrement in cracks on counter behind pizza prep table, in cracks in service pass thru from kitchen.
High Priority - Rodent activity present as evidenced by rodent droppings found.

Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Milk in reach in cooler in front counter no date mark. Employee dated milk opened 8-11 **Corrected On-Site**
Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Can opener soiled with food debris, mixer head soiled with food debris.
Intermediate - Handwash sink not accessible for employee use at all times. Hand sink in exterior bar blocked with strainer. Employee removed to wash hands. **Corrected On-Site**
