A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed acoustic tiles through out kitchen. **Repeat Violation** **Warning**
Basic - Dead roaches on premises. Observed approximately 6 dead roaches in kitchen. Approximately 2 under triple sink, 2 under handwashing sink and 2 along wall near fryer cleaner. Approximately 7 more dead roaches along wall near door way to dining room and 3 dead on lower shelf of food preparation table in kitchen with potato on it. Observed 6 semi dead roaches on sticky trap 2 door reach incooler next to fryers.1 dead in bar area behind soda syrup along wall. Approximately 3 dead in dry storage area, 1 on shelf and 2 beneath wire rack. 1 dead roaches in women's bathroom. 1 dead roaches along wall in corner next to dry storage area. **Warning**
Basic - Floor tiles missing and/or in disrepair. Observed missing floor tiles in dry storage area under wire rack. Observed missing flor tiles on cook line beneath reach-in coolers and under fryers. Observed missing floor tiles under triple sink. **Repeat Violation** **Warning**
Basic - Food stored on floor. Observed fryer oil being stored on floor. Operator moved oil to shelf. **Corrected On-Site** **Repeat Violation** **Warning**
Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed interior of all reach in cooler with food debris and soil. **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 4 live roach in corner near handwashing sink. 1 on hand wash sink in corner, 2 beneath hand wash sink, 1 between chemicals at hand wash sink. **Warning**
Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed food container on cook line beneath water next to foil sheets with build up of soil, food debris and grease. **Warning**
Intermediate - Manager or person in charge lacking proof of food manager certification. Observed no proof of certified food mangers certificate. **Warning**
Intermediate - No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed Rylee T hired over 60 days without proof of food handlers training. **Warning**
