A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises. Observed approximately 5 dead roaches inside of back of left side flip top cooler on cook line in kitchen.
Observed approximately 4 dead roaches inside of employee restroom located in back of house dry storage area in kitchen.
Observed approximately 4 dead roaches inside of drip pan of water heater in dry storage area in kitchen.
Observed 2 dead roaches inside of not in use ice bin at water station at wait station in front of kitchen.
Observed 1 dead roach inside of door of right side cook line flip top. **Warning**
Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoops improperly stored in spice containers. **Warning**
Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed on shelving in walk in cooler **Repeat Violation** **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed 3 live roaches on leg of prep table on left side cook line in kitchen. Observed approximately 5 live roaches inside of wheels of left side flip top on cook line in kitchen.

Observed approximately 5 live roaches inside of wheels of right side flip top on cook line in kitchen.
Observed approximately 4 live roaches on ground under cook line equipment flip tops in kitchen.
Observed 1 live roach inside of employee restroom located in back of house dry storage area in kitchen.
Observed 1 live roach under top ware washing shelf in ware washing area in kitchen **Repeat Violation** **Warning**
High Priority - Roach excrement and/or droppings present. Observed build up of roach droppings inside of soap dispenser at hand wash sink in dry storage area in kitchen **Warning**
Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed under cutting boards of cook line flip top coolers **Warning**
