A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed on shelf above rice and beans; cell phone and glasses. Operator moved to proper area. **Corrected On-Site** **Repeat Violation** **Warning**

Basic - Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed on prep table where cook is working. Cook moved to underneath table. Observed bucket next to hand sink on cook line, operator removed. **Corrected On-Site** **Warning**
Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed in all walk in coolers in main kitchen area with rust. **Warning**
Basic - Working containers of food removed from original container not identified by common name. Observed sugar container with no label in dry food storage. Observed flour in dry food storage area. Observed sugar at bar with no label. **Warning**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 13 live flies in onion bin and in dry dish storage room in back of restaurant. Observed 3 live flies in dry food storage. Observed 1 live fly in warewash area. **Admin Complaint**
Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed items on menu undercooked to guests order with no marking. Observed ceviche dish served raw with no indication. Observed eggs undercooked on dish with no indication. **Warning**
